2. What is your complete First and Last name (two names only):
3. What email address would you like displayed on your website:
4. What phone number would you like displayed on your website?
5. Please include whether you are part of phase 1 only or phase 1 and phase 2
6. Your sponsor's ID (if you have not purchased phase 2)
7. What is your Facebook profile or page URL:
8. Please attach a picture you would like to use and a copy of you order form (for those who would like to sign up by mail).
   Important: Your forms must be sent in .pdf format

Once your website is completed you will receive an email confirmation. Please allow up to 5 business days for your website to be completed.
